- Documents of accomplishments (optional)
- One (1) High Quality, four-color photo (head and shoulders only) of nominee in business attire (.jpg;pdf;tif;)
- Answer three of four questions of your choice below. You may write them in or complete them via video. Please start your short video stating your name, employer and nominator. Be creative and have fun with it!
